Your Practice

The full name of your affiliate organization.
Enter the 4-digit numeric year your business opened.
Enter the email address you want to receive messages at. Leave this field blank if you do not want to receive email from potential clients.
Enter the phone number for your business.
The filename you type above must exist in "/userdata/images/" to appear.

Your Information

Your name, including your credentials
Enter your 6-character AEA/CertID number here. It must match your AEA membership record.

Your AEA/CertID is used to verify your access to AEA-owned content. It is also used to generate your professional membership's list, and your credentials (if you are a CPE).

Important: This website template includes general electrolysis content that is the property of the American Electrology Association (AEA). The AEA permits its members, who are in good standing, to display this content. Your Cert/ID number will be used to verify use of this content on the following pages:
